Maine Licensed Care Manager

Job Summary

To improve member health outcomes, the full-time Maine Licensed Care Manager will provide comprehensive care management interventions remotely, engaging with members and families to assess, educate, and coordinate care while collaborating with healthcare providers and community services.

Key responsibilities

• Conduct comprehensive nursing assessments and provide care management according to established standards
• Collaborate with healthcare providers and community agencies to address member-centered goals and needs
• Document all interactions and assessments in compliance with care management standards

Required qualifications

• Bachelor's degree in nursing required; MSN preferred
• 5 years of clinical experience in relevant specialties such as home care, community health, or chronic disease management
• Unrestricted state licensed registered nurse license required
• Experience in managed care or hospital environments preferred
• CCM certification preferred
