Maine Licensed Care Manager

Job Summary

To enhance member health outcomes, the full-time Maine Licensed Care Manager will provide comprehensive, person-centered care management interventions remotely, engaging with members and families to assess, educate, and coordinate care while collaborating with various healthcare providers.

Key responsibilities:

• Conduct comprehensive nursing assessments for new and existing members, focusing on complex care needs
• Collaborate with healthcare providers and community services to advocate for member-centered goals and improve health outcomes
• Document all assessments and interventions accurately in the case management medical record in accordance with established standards

Required qualifications:

• Bachelor's degree in nursing required; MSN preferred
• 5 years of current clinical experience in areas such as medical-surgical, home care, or community health
• Unrestricted state licensed registered nurse license required
• Experience in a managed care, home health, or hospital environment
• CCM certification preferred
