Job Summary
Providing leadership in patient-centered care coordination, the per diem Maine Licensed Case Manager RN will develop safe discharge plans, facilitate transitions in care, and collaborate with multidisciplinary teams to optimize patient outcomes while working onsite in Bangor, Maine.
Key responsibilities:
- Coordinates patient care processes to achieve quality outcomes and manages resource utilization

Required qualifications:
- Associate's degree in Nursing
- Current, unrestricted compact RN license in the state of residence
