Job Summary
Providing leadership in patient-centered care coordination, the full-time Maine Licensed Case Manager RN will develop safe discharge plans and collaborate with multidisciplinary teams to ensure optimal transitions in care, all while working remotely.
Key Responsibilities
- Coordinates patient care processes to achieve desired quality outcomes and promotes continuity of care
- Facilitates communication among healthcare team members and participates in multi-disciplinary team care conferences
- Integrates assessment of post-hospital service needs and educates patients and families on discharge plans
Required Qualifications
- Associate's degree in Nursing
- Current, unrestricted compact RN license in the state of residence
- 3+ years of experience in a hospital, acute care, or direct care setting
- Intermediate level of proficiency in typing and navigating a Windows-based environment
