Job Summary

To revolutionize healthcare delivery, the full-time remote Maine Licensed Family Nurse Practitioner will conduct virtual wellness visits, perform health assessments, and provide comprehensive care coordination while working with a diverse patient population.

Key responsibilities

• Conduct virtual adult and pediatric wellness visits, including health risk assessments and medication reviews
• Collaborate with in-home clinical team members to evaluate patient needs and arrange necessary services
• Provide comprehensive care management, including preventive care interventions and referrals to specialists

Required qualifications

• Board Certified as a Family Nurse Practitioner
• Active Family Nurse Practitioner license in Maine
• Experience performing wellness visits and delivering high-quality health services
• Technologically savvy with experience using digital tools for patient care coordination
• Ability to work independently and demonstrate professionalism and compassion in patient interactions
