Job Summary

A company is looking for a Population Health Coordinator - Medical.

Key Responsibilities

• Manage data and registry for special populations, assessing progress and facilitating care team collaboration
• Coordinate care for complex members, providing direct support and training staff on collaborative care practices
• Support individual member needs through outreach, education, and crisis management while documenting treatment plans

Required Qualifications

• Registered Nurse (RN) or Licensed Practical Nurse (LPN) with SAMHSA required training for an X DEA license
• Bachelor's degree in Nursing preferred
• At least 5 years of experience in direct patient care in addiction medicine or related behavioral health fields
• Understanding of care coordination, case management, and health care regulations
• Proficiency in EMRs, Microsoft Office/Google Suite, and data analysis
