Job Summary

A company is looking for a Population Health Coordinator - Medical.

Key Responsibilities

• Manage data and registry for special populations, assessing progress and proposing quality improvement initiatives
• Coordinate care for complex members, including training staff on the Collaborative Care Model
• Provide direct member care through outreach, screenings, and education on health-related issues

Required Qualifications

• Registered Nurse (RN) or Licensed Practical Nurse (LPN) with SAMHSA required training for an X DEA license
• Bachelor's degree in Nursing preferred
• At least 5 years of experience in addiction medicine or related behavioral health fields
• Understanding of care coordination, case management, and healthcare regulations
• Access to reliable internet and telephone services for remote work
