Job Summary
Supporting high-acuity member populations, the full-time Maine Licensed Population Health Coordinator will manage data and registry tasks, coordinate care for complex cases, and provide direct member care, all while pioneering innovative care delivery methods in a collaborative environment.
Key responsibilities
- Maintain and assess an accurate registry of members, facilitating reviews and proposing quality improvement initiatives
- Perform internal and external care coordination tasks for complex members and assist in training staff on the Collaborative Care Model
- Provide direct support to assigned members through telephonic outreach, screenings, and educational interventions
Required qualifications
- Registered Nurse (RN) or Maine Licensed Practical Nurse (LPN) with SAMHSA required training for an X DEA license
- Bachelor's degree in Nursing preferred
- At least 5 years of direct patient-care experience in addiction medicine or related behavioral health fields
- Strong understanding of care coordination and case management
- Proficiency in multiple EMRs and Microsoft Office/Google Suite, with spreadsheet proficiency required
