Job Summary

A company is looking for a Social Worker / Care Management to support Internal Medicine and Family Medicine practices.

Key Responsibilities

• Provide complex care management in a primary care setting
• Assist patients in overcoming barriers to health-related goals by addressing social drivers of health
• Collaborate with a patient-centered team to improve health outcomes

Required Qualifications, Training, and Education

• Bachelor's Degree in Social Work from an accredited institution
• Active license by the Maine State Board of Social Work
• Experience providing therapeutic services to adult and pediatric patients with chronic medical issues
• Experience in client advocacy and case management
