Maine Licensed Wraparound Care Coordinator

Job Summary

A company is looking for a Wraparound Care Coordinator to provide intensive case management services to youth and families with mental health diagnoses or developmental disabilities.

Key Responsibilities

• Provide home-based and community-based case management services to youth and families
• Attend monthly staff meetings and participate in weekly supervision
• Develop individualized goals for families/youth to maintain stability in their homes and communities

Required Qualifications

• Bachelor's Degree in Social Work or related field required
• At least two years' experience working with families and children, preferably within the mental health system
• Knowledge of serious and persistent mental illness, substance abuse, and trauma
• Ability to work flexible hours, including evenings
• Must be able to pass background checks and have a good driving record
