Maine Licensed Wraparound Care Coordinator

Job Summary

A company is looking for a Wraparound Care Coordinator to provide case management services to youth and families with mental health diagnoses or developmental disabilities.

Key Responsibilities

• Provide home-based and community-based case management services
• Collaborate with a team to develop individualized goals for youth and families
• Attend monthly staff meetings and participate in weekly supervision

Required Qualifications

• Bachelor's Degree in Social Work or related field required
• At least two years' experience working with families and children, preferably in mental health
• Knowledge of serious mental illness, substance abuse, and trauma
• Ability to work flexible hours, including evenings
• Must have a good driving record and meet physical requirements for family visitation
