Job Summary

A company is looking for a Care Coordinator (Remote) to support members with chronic and complex health needs.

Key Responsibilities

• Conduct telephonic outreach to members, caregivers, and providers for appointment confirmation and care plan support
• Assist nurses and care managers by gathering updates, verifying services, and documenting interactions
• Coordinate medical appointments and follow up with members after hospital discharges to ensure appropriate services

Required Qualifications

• Active certification as a CNA, LNA, CHHA, CMA, or Medical Assistant
• Minimum 1 year of experience in healthcare settings
• Comfortable with electronic documentation systems and basic Microsoft Office tools
• Able to work independently in a remote environment while managing multiple tasks
