Job Summary

A company is looking for a Care Manager (Remote).

Key Responsibilities

• Identify members with acute/complex medical and/or behavioral health conditions and develop comprehensive care plans
• Collaborate with members and the interdisciplinary care team to monitor and update care plans, ensuring alignment with medical benefits
• Document member data according to application protocols and regulatory standards

Required Qualifications

• High School Diploma or GED required; Bachelor's degree in nursing preferred
• CCM/ACM or other RN Board Certified certification in case management preferred
• 5 years of clinically related experience in Care Management or direct patient care
• RN - Registered Nurse - State Licensure required upon hire
• Must be eligible to work in the U.S. without sponsorship
