Maryland Licensed Care Manager
Job Summary
To support client care initiatives, the remote Maryland Licensed Care Manager will develop and implement individualized care plans, coordinate community services, and advocate for patients' needs while ensuring compliance with healthcare policies.
Key responsibilities
- Develop and initiate comprehensive Individual Care Plans in accordance with program guidelines
- Coordinate patient care through assessment, evaluation, and collaboration with healthcare teams
- Manage discharge plans and act as a patient advocate regarding insurance and financial assistance
Required qualifications
- Licensed Practical Nurse (LPN) credential required
- Two years of utilization management experience with evidence-based clinical criteria
- Three years of clinical experience, preferably in case management or a related area
- Bachelor's degree in nursing or a related health services field preferred
- Certified Case Manager (CCM) certification preferred
