Maryland Licensed Case Manager

Job Summary

To support a growing healthcare team, the full-time Maryland Licensed Case Manager will coordinate care and health education for patients with chronic conditions while working remotely during standard business hours.

Key responsibilities

• Identifies and acts upon impending changes in patient conditions
• Analyzes issues impacting patient care and initiates corrective actions
• Maintains budgeted caseload and makes decisions based on established guidelines

Required qualifications

• Current Maryland licensure as a registered nurse or LCSW-C
• Bachelor's degree in nursing or related field, or MSW in social work preferred
• Minimum five years of clinical practice experience
• Three years of clinical practice in a specific specialty preferred
• Certification in case management (CCM) preferred
