Job Summary

Coordinating and facilitating care for targeted member populations, the full-time Maryland Licensed Clinical Social Worker will manage complex case management processes for individuals with chronic conditions or high-cost specialty care needs while working remotely.

Key responsibilities

• Coordinate care and health education for members with chronic conditions
• Apply advanced knowledge of case management processes to achieve member goals
• Maintain thorough documentation of outcomes and member progress

Required qualifications

• Master's Degree in Social Work
• Minimum of three years of clinical practice experience
• At least three years of case management experience
• Managed care experience preferred
