Job Summary

A company is looking for a Hospital Coordinator & Case Manager for the Readmissions Reduction Program.

Key Responsibilities

• Oversee and coordinate the Readmissions Reduction Program, ensuring effective communication with hospital staff
• Serve as a case manager for clients with mental illness and/or substance use disorders, providing support and guidance
• Manage data entry and documentation related to patient intake and care coordination

Required Qualifications

• Bachelor's Degree in psychology, social work, or a related behavioral health discipline
• 2-4 years of relevant experience in behavioral health, including case management and crisis intervention
• Knowledge of community resources and benefits related to mental health and substance use disorders
• Proficiency in Google Suite and Microsoft Office software
• Ability to work in a fast-paced environment and manage multiple tasks effectively
