Job Summary

Seeking a Maryland Licensed Nurse Practitioner with cardiology experience, the full-time position will manage patients in an outpatient virtual transition of care heart failure clinic, focusing on comprehensive evaluations, guideline-directed medical therapy, and collaboration with an interdisciplinary team.

Key Responsibilities

• Virtually perform comprehensive assessments and manage heart failure patients according to clinical guidelines
• Collaborate with cardiologists and primary care providers to ensure seamless transitions of care and timely follow-ups
• Provide education to patients and families on heart failure management and medication adherence

Required Qualifications

• 1 year of cardiology experience, preferably in heart failure
• Acute, family, or primary care certification
• BLS certification
• Ability to work collaboratively in an interdisciplinary team
• Experience with virtual patient management and care coordination
