Job Summary
To support healthcare operations, the full-time Maryland Licensed Outreach Coordinator will manage member inquiries, assess health and social risk factors, and coordinate care plans while working remotely.
Key responsibilities
- Respond to prospective member and provider inquiries generated through marketing initiatives
- Assess members' health and social needs to develop and coordinate appropriate care plans
- Facilitate member enrollment and ensure connections to necessary healthcare services and resources
Required qualifications
- High School diploma or equivalent; Associate's degree in advocacy, healthcare, or community relations preferred
- Basic knowledge of social and health issues relevant to low/moderate income families
- Experience engaging with community stakeholders and advocating for underserved groups
