Job Summary
Working remotely, the full-time Maryland Licensed RN Case Manager will coordinate post-discharge care for members transitioning from acute care, conducting assessments, collaborating with care teams, and ensuring effective follow-up to prevent hospital readmissions.
Key responsibilities
- Initiate outreach and assess members' needs by collaborating with hospital case managers and conducting intake evaluations
- Develop and implement post-discharge care plans in collaboration with the care team, addressing both clinical and non-clinical needs
- Perform regular follow-ups and check-ins with members, ensuring proper medication reconciliation and tracking care metrics
Required qualifications
- Maryland RN license
- Graduate of an accredited school of nursing
- 3+ years of nursing experience, preferably in case management or transitional care
- Experience with behavioral health assessment tools and care coordination
- Proficient in electronic health records and care facilitation platforms
