Job Summary

A company is looking for a Palliative Care Outpatient Telehealth Social Worker.

Key Responsibilities

• Conduct comprehensive psychosocial assessments via telehealth for patients with serious illness and their caregivers
• Provide counseling and emotional support to patients and families coping with illness-related challenges
• Facilitate discussions on goals of care and advance care planning while collaborating with the interdisciplinary palliative care team

Required Qualifications

• Master's degree in social work from an accredited institution
• Minimum of 2 years of licensed clinical social work experience in a mental health or healthcare setting
• Must be licensed at the LCSW-C/LICSW level in MD and DC
• Advanced palliative hospice social worker certification (APHSW-C) preferred
• Knowledge of palliative care principles and community resources relevant to patients with complex medical needs
