Maryland Licensed Transitional Case Manager

Job Summary

A company is looking for a Transitional Case Manager to facilitate patient transitions from facility settings to post-acute care.

Key Responsibilities:

• Educate patients on post-discharge follow-up and homebound criteria
• Assess readmission risk and coordinate ancillary services as needed
• Liaise between the agency and healthcare providers to ensure continuity of care

Required Qualifications:

• Current, unrestricted RN, LPN, or SW licensure in state of practice
• Current CPR certification
• 1+ years of home health or hospital case management experience
• Current driver's license and access to a dependable vehicle or public transportation
