Massachusetts and New Hampshire RN Complex Care Manager

Job Summary

To support patient care in a remote setting, the part-time RN Complex Care Manager will coordinate healthcare services, manage client assessments and treatment plans, and communicate with third-party payers for service authorizations.

Key responsibilities

• Coordinate and support healthcare within the facility and referrals for external services
• Modify patient treatment plans based on their responses and prepare patients/families for discharge
• Maintain accurate reports and collaborate with medical directors on challenging cases

Required qualifications

• Associate's Degree in Nursing required; Bachelor's Degree preferred
• Registered Nurse (RN) License for Massachusetts and New Hampshire required
• 2+ years of experience in case management, utilization review, or discharge planning preferred
