Job Summary
Collaborating with an inter-disciplinary care team, the full-time Remote Massachusetts Behavioral Health Case Manager will engage members, develop individualized care plans, and coordinate discharge planning for individuals receiving behavioral health services.
Key responsibilities:
- Engage members to complete health and psychosocial surveys while establishing effective communication
- Develop and coordinate individualized care plans in collaboration with facility staff and the care team
- Document care coordination activities and evaluate member satisfaction through open communication
Required qualifications:
- Bachelor's Degree in a healthcare-related field or equivalent relevant work experience
- One year of experience in case management or care coordination for individuals with behavioral health conditions
- Three years of experience in a healthcare field related to discharge planning or care management
- Intermediate proficiency in Microsoft Office, including Outlook, Word, and Excel
- Knowledge of local, state, and federal healthcare laws and regulations related to case management
