Massachusetts Care Coordinator

Job Summary

Supporting a growing healthcare initiative, the full-time remote Massachusetts Care Coordinator will perform telephonic outreach and assessments, coordinate community resources, and advocate for member access to essential services while engaging members in care management activities.

Key responsibilities:

• Conducts interviews and structured assessments using Motivational Interviewing techniques to identify member needs and engage them in care management
• Facilitates communication between members and external partners, ensuring access to necessary resources and services
• Maintains documentation in the medical management information system, adhering to performance metrics and internal policies

Required qualifications:

• Associate's degree or High School Diploma/equivalent with relevant work experience
• Two years of office experience in a high-volume data entry, customer service call center, or healthcare administration
• Knowledge and experience in maternal child health preferred
• Experience with healthcare databases such as FACETS/JIVA is desirable
• Successful completion of pre-employment background check
