Job Summary

Massachusetts Licensed Care Coordinator, a full-time remote position responsible for facilitating smooth transitions of care for members, ensuring timely communication among providers, and addressing barriers to care.

Key Responsibilities

• Ensure timely resolution of transitional needs and barriers to meet regulatory timeframes
• Interact with providers and members to gather clinical information and understand transition needs
• Track long-stay cases and coordinate communication regarding discharge needs among stakeholders

Required Qualifications

• Associate's Degree in Nursing or Bachelor's Degree in Healthcare Administration or a related field required
• Massachusetts Registered Nurse (RN) license required
• 2-3 years of experience in health plan clinical services/utilization management required
• 2-3 years of clinical experience required
