Massachusetts Licensed Care Coordinator

Job Summary

To support proactive care coordination for patients with complex chronic conditions, the fully remote Massachusetts Licensed Care Coordinator will manage patient outreach, maintain care plans, and ensure compliance with documentation and billing requirements while working within a structured, high-volume environment.

Key responsibilities

• Support patient enrollment and eligibility verification for Chronic Care Management (CCM) programs
• Conduct weekly patient outreach through text and phone calls to reinforce care plans and ensure engagement
• Document patient interactions accurately and maintain compliance with HIPAA and billing standards

Required qualifications

• Certified Medical Assistant
• 1-3 years of healthcare experience, preferably in chronic disease or nephrology
• Strong clinical assessment and documentation skills
• Ability to manage a structured workflow in dual systems (EHR and CCM platform)
• Intermediate proficiency with MS Office Suite, including Outlook and Teams
