Job Summary

A company is looking for an MTM Pharmacist Independent Contractor.

Key Responsibilities

• Conduct patient outreach through phone calls to provide medication therapy management services
• Offer expert support and guidance through comprehensive medication reviews and counseling on medical device usage
• Assess medication therapy and document clinical recommendations and patient care notes

Required Qualifications

• Doctor of Pharmacy (Pharm.D.) or Bachelor of Science in Pharmacy (BS Pharm) degree
• Active pharmacist license to practice in Massachusetts
• Ability to obtain licensure in additional states as needed
• Strong telephonic skills for effective communication with patients
• Experience in conducting Medication Therapy Management services is preferred
