Job Summary

To support the ACCESS program, the part-time Massachusetts Licensed Nurse Practitioner will conduct virtual medical evaluations, create evidence-based care plans, and collaborate with the Medical Director to build clinical workflows and protocols in a fully remote environment.

Key responsibilities
  • Conduct virtual medical evaluations and document patient interactions in the EHR
  • Create and manage holistic care plans for patients with cardiometabolic conditions
  • Collaborate with the Medical Director to develop clinical protocols and scalable workflows
Required qualifications
  • Active NP license in good standing; IMLC/eNLC compact license preferred
  • Board certification through ANCC or AANP as a Family, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care NP
  • Minimum 2 years of post-licensure clinical experience in relevant fields
  • Demonstrated comfort with telehealth and digital care delivery
  • Strong documentation discipline and familiarity with EHR systems
