Job Summary
To support the ACCESS program, the part-time Massachusetts Licensed Nurse Practitioner will conduct virtual medical evaluations, create evidence-based care plans, and collaborate with the Medical Director to build clinical workflows and protocols in a fully remote environment.
Key responsibilities
- Conduct virtual medical evaluations and document patient interactions in the EHR
- Create and manage holistic care plans for patients with cardiometabolic conditions
- Collaborate with the Medical Director to develop clinical protocols and scalable workflows
Required qualifications
- Active NP license in good standing; IMLC/eNLC compact license preferred
- Board certification through ANCC or AANP as a Family, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care NP
- Minimum 2 years of post-licensure clinical experience in relevant fields
- Demonstrated comfort with telehealth and digital care delivery
- Strong documentation discipline and familiarity with EHR systems
