Job Summary
To support a growing Advanced Therapeutics Program, the full-time Massachusetts Licensed Nurse Practitioner will manage patients with obesity, diabetes, and cardiometabolic conditions through comprehensive telehealth visits in a fully remote setting.
Key responsibilities
- Conduct comprehensive telehealth visits and manage advanced therapeutics for patients with chronic conditions
- Collaborate with a multidisciplinary care team to optimize patient outcomes and develop new care pathways
- Monitor laboratory results, adjust medications, and educate patients on lifestyle modifications and medication adherence
Required qualifications
- Current Massachusetts NP licensure and Master's degree
- Certification from an accredited Nurse Practitioner program
- Minimum of 2+ years of experience managing chronic conditions
- Experience prescribing and managing advanced therapeutics
- Comfort working in a fully virtual care environment
