Job Summary
To support a growing Advanced Therapeutics Program, the part-time per diem Massachusetts Licensed Nurse Practitioner will conduct telehealth visits for patients with obesity, diabetes, and cardiometabolic conditions, manage advanced therapeutics, and collaborate with a multidisciplinary care team in a fully remote environment.
Key responsibilities:
- Conduct comprehensive telehealth visits for patients managing obesity, diabetes, and cardiometabolic conditions
- Initiate and manage advanced therapeutics, adjusting treatment plans based on laboratory results
- Collaborate with a multidisciplinary team to educate patients on lifestyle modifications and medication adherence
Required qualifications:
- Current Massachusetts NP or PA licensure and Master's degree
- Certification from an accredited Nurse Practitioner or Physician Assistant program
- BLS and ACLS certification
- Minimum of 2+ years of experience managing chronic conditions in an APP role
- Experience prescribing and managing advanced therapeutics
