Job Summary

To enhance patient access and streamline treatment, the full-time Massachusetts Licensed Nurse Practitioner will provide remote clinical care to patients with neurological conditions and memory disorders, collaborating closely with physicians and the care team.

Key responsibilities

• Deliver direct clinical care via telehealth to patients with neurological and memory disorders
• Conduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians
• Educate patients and families on diagnoses, treatment plans, and disease progression while maintaining accurate clinical documentation

Required qualifications

• Active, unrestricted Nurse Practitioner license in Massachusetts; certification in geriatrics, adult-gerontology, or family practice preferred
• Minimum of 2 years of clinical experience in neurology, memory care, or dementia care settings
• Strong clinical judgment and documentation skills
• Ability to manage a full patient panel independently with physician collaboration
• Excellent communication skills with patients facing cognitive decline and their caregivers
