Job Summary
To enhance patient access and streamline treatment, the full-time Massachusetts Licensed Nurse Practitioner will provide remote clinical care to patients with neurological conditions and memory disorders, collaborating closely with physicians and the care team.
Key responsibilities
- Deliver direct clinical care via telehealth to patients with neurological and memory disorders
- Conduct patient assessments, develop care plans, and manage ongoing treatment in collaboration with supervising physicians
- Educate patients and families on diagnoses, treatment plans, and disease progression while maintaining accurate clinical documentation
Required qualifications
- Active, unrestricted Nurse Practitioner license in Massachusetts; certification in geriatrics, adult-gerontology, or family practice preferred
- Minimum of 2 years of clinical experience in neurology, memory care, or dementia care settings
- Strong clinical judgment and documentation skills
- Ability to manage a full patient panel independently with physician collaboration
- Excellent communication skills with patients facing cognitive decline and their caregivers
