Job Summary
To support a growing palliative care team, the full-time Massachusetts Licensed Palliative Care Nurse will manage patient care delivery, build relationships with patients and families, and conduct telephone triage in a remote environment.
Key responsibilities
- Build therapeutic relationships with patients and families, providing education and managing care transitions
- Deliver patient care in alignment with RN protocols, including medication administration and chronic disease management
- Conduct telephone triage and support ongoing care coordination with the clinical team
Required qualifications
- Graduate of a State-approved school of nursing
- Current, unrestricted license to practice professional nursing in the Commonwealth of Massachusetts
- American Heart Association Basic Life Support (BLS) certification
- Proven up-to-date knowledge of current standards of nursing care in specialty
- Experience with electronic medical records (EMR) and proficiency in word processing and spreadsheet programs
