Massachusetts Licensed Population Health Nurse Practitioner

Job Summary

A company is looking for a Population Health Nurse Practitioner (Remote, MA License).

Key Responsibilities

• Provide follow-up care for patients post-hospital discharge through telephonic outreach and scheduled video/phone visits
• Conduct medication reconciliation, symptom screening, and coordinate necessary follow-up services
• Collaborate with the nursing team and maintain accountability to individual performance targets while contributing to team-level population health goals

Required Qualifications

• Certified Nurse Practitioner in Adult, Family, or Geriatrics
• Active state licensure in MA and/or CT
• 5+ years of post-graduate clinical experience in Primary Care or Geriatrics
• Direct experience engaging Medicare and Medicaid populations managing complex conditions
• Comfort in a digital-first environment with solid tech proficiency
