Job Summary

To enhance patient care through innovative technology, the full-time Massachusetts Licensed Practical Nurse (LPN) Remote Nurse Monitor will engage with patients, monitor health data, and provide education and support while working remotely from Boston, MA.

Key Responsibilities

• Introduce new patients to the remote monitoring program and ensure their comfort with device usage
• Review biometric data and patient-reported information to identify changes in health conditions
• Engage with patients for education and early intervention when health readings are concerning

Required Qualifications

• Active and unrestricted LPN/LVN license in Massachusetts, with willingness to obtain licensure in Connecticut and New York
• Strong understanding of clinical decision-making for remote patient monitoring
• Experience in primary care, chronic disease management, or population health preferred
• Proficiency with EMR/EHR systems and digital tools for managing patient data
• Minimum of 3 years of practical nursing experience in relevant settings preferred
