Job Summary

A company is looking for a Licensed Practical Nurse (LPN) with Massachusetts Licensure required.

Key Responsibilities

• Manage patient census to close gaps in clinical and non-clinical patient care
• Identify and coordinate community resources for patient care
• Provide education on chronic condition management and perform medication management

Required Qualifications and Education

• Active Massachusetts License (LPN)
• At least 3 years of direct patient-facing work experience
• Technical aptitude with Microsoft Office Suite
• Experience with medication management and chronic care coordination
• Ability to work in a remote, HIPAA-compliant workspace
