Job Summary

A company is looking for a Licensed Practical Nurse- Advanced Therapeutics for a remote position.

Key Responsibilities:

Conduct telephonic and video-based patient triage to assess symptoms and clinical concerns
Provide comprehensive patient education on medications and lifestyle interventions
Document clinical encounters and coordinate care with clinical and administrative teams

Required Qualifications:

Current and active LPN license
Minimum 2 years of clinical experience in ambulatory, primary care, specialty, or telehealth
Ability to work independently in a remote environment
Proficiency with telehealth technologies
Familiarity with electronic medical record systems, preferably Epic
