Massachusetts Licensed RN Case Manager

Job Summary

A company is looking for a Transition of Care RN Case Manager - Part-Time.

Key Responsibilities

• Conduct client assessments and modify treatment plans based on patient responses
• Prepare patients and families for discharge and communicate with third-party payers for service authorization
• Maintain accurate records and review challenging cases with medical directors as needed

Required Qualifications

• Associate's Degree in Nursing (ADN) or Bachelor's Degree in Nursing (BSN)
• Active and unrestricted RN Licensure
• Background in Care Management and/or Home Care
• At least 2 years of experience in case management, utilization review, and/or discharge planning
• Understanding of diagnostic criteria for dual conditions and knowledge of Healthcare and Managed Care is preferred
