Key Responsibilities

• Collaborate with healthcare professionals to deliver social work case management services
• Assess and address members' psychosocial needs while coordinating care and resources
• Document interactions and maintain compliance with accreditation standards and policies

Required Qualifications and Education

• Master's degree in Social Work, Psychology, Education, Public Health, or related field
• Current state licensure as a social worker or in a related field
• Minimum of three years of experience with individuals having complex medical, behavioral, and psychosocial needs
• Experience in patient education and care management
• Knowledge of chronic health conditions is preferred
