Job Summary

A company is looking for a Triage Nurse Specialty Navigator for its Contact Center.

Key Responsibilities

• Facilitate patient access to appropriate care through telephone triage and coordination of services
• Conduct patient assessments, research, and collaborate with healthcare providers and staff
• Provide medical information and education to patients and families regarding treatment and services

Required Qualifications, Training, and Education

• Massachusetts RN license and BLS CPR certification
• Minimum of 5 years of nursing experience, preferably in GI or Primary Care Triage
• Experience in patient triage and care coordination
• Associates of Science in Nursing required; Bachelor's Degree in Nursing preferred
• Proficient in electronic medical records and multiple computer applications
