Job Summary

A company is looking for a Care Team Assistant.

Key Responsibilities

• Manage incoming and outgoing patient calls, performing protocol-based triage and documenting phone notes
• Ensure all pre-visit preparations are completed by reviewing patient schedules and obtaining necessary medical documents
• Assist care team with form completion, patient reports, and referrals for vulnerable populations

Required Qualifications

• High-school level education or equivalency
• Medical Assistant Certification or equivalent, or at least 2 years of clinical experience in an outpatient setting
• Spanish fluency preferred
