Medical Assistant Care Navigator

Job Summary

Providing essential clinical and administrative support, the full-time remote Medical Assistant Care Navigator will manage patient communication, assist with electronic health record tasks, and coordinate care to enhance member experiences and improve health outcomes.

Key responsibilities

• Answer incoming clinical calls and resolve patient needs while managing follow-up communications
• Perform chart preparation and manage electronic health record tasks, ensuring accuracy and timely follow-up
• Collaborate with care teams to close gaps in care, including managing referrals and coordinating lab results

Required qualifications

• High school diploma or equivalent required; completion of a Medical Assistant program preferred
• Certification as a Medical Assistant (CMA, RMA, CCMA, or equivalent) preferred
• 2-5 years of experience as a Medical Assistant in a primary care or urgent care setting
• Experience with eClinicalWorks (eCW) or comparable electronic health record systems strongly preferred
• Experience with telehealth or remote patient care support is preferred
