Job Summary

A company is looking for a Medical Director - Post-Acute Care Management - Care Transitions - Remote.

Key Responsibilities

• Provide daily utilization oversight and communicate with network physicians and hospitals
• Conduct clinical case reviews and peer-to-peer conversations as needed
• Collaborate with the Client Services Team to ensure coordinated delivery of care services

Required Qualifications

• Board certification as an MD, DO, or MBBS with an unrestricted medical license
• Ability to obtain licensure in multiple states
• 3+ years of post-residency patient care experience, preferably in inpatient or post-acute settings
