Job Summary

A company is looking for a Medicare Coach to join their Medicare Member Operations team.

Key Responsibilities:

• Serve as the primary contact for Medicare members and caregivers, providing empathetic support and coordinating comprehensive care
• Administer health risk assessments and coach members to create vision statements and long-term health goals
• Collaborate with various specialists and healthcare providers to ensure tailored interventions and support for members

Qualifications:

• 2-3 years of experience in care/case management or health coaching, particularly with Medicare members
• Preferred: Certification in health coaching, RN, MSW, or relevant clinical licensure
• Knowledge of Medicare benefits and healthcare resources
• Proficient in using digital tools and platforms for communication and documentation
• Comfortable with proactive outreach and engaging members via phone
