Medication Navigator

Job Summary

To enhance patient adherence to prescribed medications, the full-time Medication Navigator will engage with patients, providers, and pharmacies while working remotely to identify and overcome barriers to medication access and usage.

Key responsibilities:

• Engage with patients and healthcare providers to promote timely medication fills and refills, particularly for chronic conditions like cholesterol, hypertension, and diabetes
• Educate patients on their medication regimens and assist in resolving issues related to adherence
• Track outreach efforts and performance metrics, and communicate with leadership to escalate issues and support solutions

Required qualifications:

• High School Diploma or GED
• 1+ years of experience working with patients, preferably in medication reconciliation or management
• 1+ years of experience with Microsoft Office products, including Word and Outlook
• Ability to work Monday through Friday from 8:30 am to 5:00 pm EST
• Access to a designated quiet workspace at home with secure handling of Protected Health Information (PHI)
