Member Care Coordinator

Job Summary

A company is looking for a Member Care Coordinator for their remote, full-time position in the operations department.

Key Responsibilities:

• Facilitate enrollment and discharges into and out of the organization
• Manage clinical care transitions and coordinate mental health services
• Develop relationships with outside service providers and ensure compliance with care coordination

Required Qualifications:

• Master's degree in a related field
• Unrestricted state licensure preferred (LCSW or LPC)
• 2-3 years of post-licensure experience
• Experience in care coordination or case management in a mental health or behavioral health setting
• Bilingual English/Spanish preferred
