Member Health Assessor

Job Summary

A company is looking for a Member Health Assessor with a 4-day weekday and 1-day weekend schedule.

Key Responsibilities

• Engage with members to conduct health risk assessments and establish professional relationships
• Coordinate logistics to support care plan goals and verify member eligibility and health status
• Assist in the development of care plans and provide health education and wellness materials

Required Qualifications

• High School Diploma or General Education Diploma (GED) is required
• A minimum of two (2) years of experience working with at-risk populations in community settings is preferred
• Proficient in Microsoft Office, including Outlook, Word, and Excel
• Experience working within diverse cultures and with various stakeholders
• Ability to maintain confidentiality and demonstrate ethical conduct in all interactions
