Job Summary

A company is looking for a Member Health Assessor.

Key Responsibilities

• Conduct health risk assessments by gathering information from members and stakeholders
• Coordinate logistics to support care plan goals and assist with health education and wellness materials
• Maintain documentation and contribute to the development of care plans based on assessment outcomes

Required Qualifications, Training, and Education

• High School Diploma or General Education Diploma (GED) is required
• Minimum of two (2) years of experience working with at-risk populations in community settings is preferred
• Proficiency in Microsoft Office, including Outlook, Word, and Excel
• Experience working within diverse cultures and sensitivity to their needs
• Ability to maintain confidentiality and demonstrate ethical conduct
