Job Summary

A company is looking for a Metabolic Health Nurse Practitioner (NP).

Key Responsibilities

• Provide direct patient care for individuals with metabolic conditions through telehealth visits
• Collaborate with registered dietitians and physicians to create comprehensive care plans
• Manage medications, order labs, track patient outcomes, and participate in quality improvement efforts

Qualifications & Requirements

• Active and unrestricted Nurse Practitioner license; multi-state licensure is a plus
• Board certification as a Family Nurse Practitioner, Adult-Gerontology Primary Care NP, or equivalent
• Minimum of 2 years of experience managing patients with metabolic conditions
• Strong foundation in evidence-based lifestyle and medical management of chronic disease
• Comfort with telehealth care delivery and modern digital health platforms
