Job Summary

To support the expansion of the Collaborative Care integrated behavioral health program, the full-time Michigan Licensed Behavioral Health Clinician will provide high-quality patient support through individual assessments, treatment plan development, and psychotherapy, while working remotely to coordinate care for patients with behavioral health needs and chronic health conditions.

Key responsibilities
  • Screen and assess patients for mental health and substance abuse disorders, facilitating treatment plans and follow-up care
  • Provide brief behavioral interventions using evidence-based techniques and support psychotropic medication management
  • Participate in weekly caseload consultations with the psychiatric consultant and document patient progress in the EHR
Required qualifications
  • A minimum of a master's degree in social work, psychology, counseling, rehabilitation, or a related field
  • 3-5+ years of experience in behavioral health intervention clinical work, case management, or community care coordination
  • Active independent clinical license in Michigan (LMSW, LPC, LMFT, or Licensed Psychologist)
  • Current enrollment in the state's Medicaid & Medicare is preferred
  • Proficiency in utilizing electronic health records (EHR) and remote communication systems
